Helping Home Health Agencies Achieve Excellence

Home health is a valuable asset that allows Medicare consumers to remain in the comfort of their homes while dealing with an illness. Through at-home visits and the use of telehealth (monitoring patients at a distance by using telephones, computers, and other information technologies), home health agencies help maintain patient independence and avoid unnecessary hospitalizations by providing the right care to the right patient at the right time.

Delmarva Foundation works with home health agencies throughout Maryland and the District of Columbia to help them improve the quality of care they provide to Medicare consumers.

Together, Delmarva Foundation and the home health agencies focus on:

  • The Centers for Medicare & Medicaid Services’ (CMS) home health quality improvement initiative, including 11 publicly reported measures:
    • Improvement in bathing
    • Improvement in dyspnea
    • Improvement in urinary incontinence
    • Improvement in ambulation/locomotion
    • Improvement in transferring
    • Improvement in management of oral medications
    • Improvement in pain interfering with activity
    • Acute care hospitalization
    • Discharge to community
    • Any emergent care provided
  • Reducing the rate of acute care hospitalizations
  • Improving the management of oral medications
  • Increasing the number of immunizations given
  • Culture change in the home health agency environment.
