Quality Awards for Home Health Agencies

Delmarva Foundation Improvement Award

The purpose of this award is to recognize individual Home Health Agency’s performance improvement based on the following

Criteria

• Setting a target for Acute Care Hospitalization (ACH)
• ACH absolute improvement of > 25%
• Completing baseline immunization survey

Delmarva Foundation Quality Award

The purpose of this award is to recognize home health agencies that have among the lowest Acute Care Hospitalization rates nationwide.

Criteria

• Acute Care Hospitalization (ACH) rate within the top 5th percentile of all agencies nationwide (publicly reported data available August 200

Senior Leadership Award

This award, chosen by a panel of Delmarva Foundation judges, recognize excellence in quality initiatives and transformational change by senior level healthcare administrators who have shown innovation, dedication, and excellence in their quality improvement initiatives over the past year.

Honorees will represent the highest levels of performance by demonstrating:

• Leadership best practices as they relate to quality and transformational change
• Ongoing collaboration other organizations to promote quality improvement
• The development and implementation of an action plan for which applies innovative concepts and best practices within their own workplaces

Only winning facilities of the annual Delmarva Foundation Quality Awards will receive the opportunity to nominate their senior leader/administrator for this award.  Awards are given in the following categories:

• Nursing Home
• Home Health

One winner per category will be chosen.
