MPSC Handoffs and Transitions Learning Network
4^th Series of Sharing Calls

We have scheduled our fourth series of Sharing Calls to be held on April 24^th, 25^th, and 28^th. Each call will center on a different type of handoff process. We ask that you register for the call that corresponds with the process your team has been working on. However, feel free to join any call that interests your team. Select the call from the following dates and times:

2 - Unit to Unit	Thursday, April 24th	2:00 p.m.
3 - To Ancillary Services	Friday, April 25th	10:00 a.m.
4 - Shift to Shift	Monday, April 28th	10:00 a.m.
5 - To Outside Providers/Home	Monday, April 28th	2:00 p.m.

If you are interested in joining more than one call, please feel free to do so. We will provide call summaries for those who are interested in multiple topics but not able to attend the calls.

After registering for the calls you will receive a confirmation e-mail with the call-in number.
